Abstract

Asthma is a common chronic inflammatory disease of the airways characterized by variable respiratory symptoms and variable expiratory airflow limitation. It affects individuals across all age groups and remains one of the most common chronic diseases of childhood. The disease is associated with recurrent episodes of wheezing, cough, chest tightness and shortness of breath, with symptom severity varying over time and between individuals.

The global burden of asthma remains substantial. The World Health Organization reports that approximately 363 million people were affected by asthma in 2023, with approximately 442,000 deaths. A disproportionate burden of asthma-related mortality occurs in low- and lower-middle-income settings, where underdiagnosis, limited access to inhaled medicines and inadequate long-term disease control remain important challenges.

Modern asthma management has evolved from a symptom-relief-centered approach toward a strategy emphasizing anti-inflammatory treatment, risk reduction, prevention of exacerbations, assessment of asthma control, optimization of inhaler technique and personalized therapy. This review integrates evidence-based respiratory medicine with supportive lifestyle and holistic wellness principles while maintaining the central principle that medical treatment remains the foundation of asthma care.

The proposed HWRi–ASTHMA Integrated Care Model™ is structured around six principles: ASSESS → IDENTIFY → CONTROL → PROTECT → OPTIMIZE → EMPOWER. The objective is not to replace evidence-based pulmonology, but to strengthen long-term asthma management through a comprehensive whole-person approach.

Introduction

Asthma is a chronic respiratory disease characterized by inflammation and narrowing of the airways. Typical manifestations include wheezing, cough, chest tightness and shortness of breath.

Modern research recognizes asthma as a complex and heterogeneous disorder involving chronic airway inflammation, bronchial smooth muscle constriction, airway hyperresponsiveness, mucus hypersecretion, variable airflow limitation and, in some individuals, structural airway remodeling. Poorly controlled asthma may result in emergency visits, hospitalization, sleep disturbance, reduced exercise capacity, absenteeism, psychological distress and reduced quality of life. Therefore, modern management should aim beyond temporary symptom relief.

Understanding Asthma

Asthma involves inflammation and narrowing of the airways. The principal pathological processes include airway inflammation, bronchoconstriction, mucus hypersecretion, airway hyperresponsiveness and, in some individuals, airway remodeling. The interaction of these processes produces variable airflow obstruction and characteristic respiratory symptoms.

Normal Airway

Open lumen, minimal inflammation, normal mucus production, and relaxed airway smooth muscle ensuring seamless airflow.

Asthmatic Airway

Demonstrates chronic inflammation, swelling of the airway lining, contraction of bronchial smooth muscle, and excess mucus accumulation, resulting in a narrowed lumen and increased resistance to airflow.

Pathophysiology of Asthma

Asthma is a heterogeneous inflammatory disease involving interactions among immune cells, airway epithelial cells, bronchial smooth muscle, inflammatory mediators, environmental exposures and genetic susceptibility.

7.1 Airway Inflammation

Involves eosinophils, mast cells, T lymphocytes, macrophages and neutrophils, contributing to airway swelling and increased sensitivity.

7.2 Bronchial Hyperresponsiveness

Airways respond excessively to allergens, smoke, cold air, exercise, infections and chemical irritants.

7.3 Bronchoconstriction & Mucus

Smooth muscle contraction narrows the airway while excess mucus obstructs airflow. Bronchodilators target constriction; anti-inflammatories target inflammation.

7.4 Airway Remodeling

Long-standing inflammation contributes to structural changes including smooth-muscle hypertrophy and subepithelial fibrosis.

08 / Acute Exacerbations

Mechanism of an Asthma Attack

An asthma attack develops through exposure to a trigger followed by immune activation, inflammatory mediator release, airway swelling, bronchoconstriction, mucus production and progressive reduction of airflow. Severe attacks can become life-threatening and require urgent medical management.

Diagnosis and Clinical Assessment

Diagnosis requires a combination of clinical history and objective assessment where possible. Typical symptoms include wheezing, shortness of breath, chest tightness and cough that vary over time and worsen at night, during exercise, with infections or after allergen exposure.

Diagnostic Tools

Spirometry for airflow limitation and reversibility assessment, peak expiratory flow monitoring, bronchodilator responsiveness, symptom pattern analysis, and evaluation of inhaler technique and adherence.

Asthma Severity and Asthma Control

Asthma severity refers to the intensity of treatment required to achieve control, whereas asthma control evaluates how well symptoms and future risks are managed (daytime symptoms, night awakenings, activity limitation, reliever requirement, and exacerbation history). Poor control should prompt systematic review.

There is no single universal 'asthma diet'. Recommendations focus on overall health, healthy body weight, nutrient-dense foods, avoidance of established individual food triggers, and avoidance of unnecessary restrictive diets. Weight management supports respiratory and metabolic health where obesity coexists.

Physical Activity and Functional Wellness

Appropriately controlled asthma should not lead to avoidance of physical activity. Individualized activity (walking, cycling, swimming, or structured exercise) supports cardiovascular fitness, muscle strength, functional capacity, and psychological wellbeing.
