Abstract

Metabolic health refers to the efficient regulation of energy intake, storage and expenditure, and its disruption underlies one of the most significant chronic disease burdens worldwide. Metabolic Syndrome — a cluster of central obesity, elevated blood pressure, elevated fasting glucose, elevated triglycerides and low HDL cholesterol — affects an estimated 12.5% to 31.4% of the global adult population, depending on diagnostic criteria applied, based on a pooled analysis of over 28 million adults across 1,129 studies (Noubiap et al., 2022).

This paper examines metabolism and metabolic health from a holistic wellness perspective while maintaining the primacy of evidence-based diagnosis and appropriate medical care. It reviews global prevalence statistics, the physiology of energy homeostasis, the components and diagnostic criteria of Metabolic Syndrome, diagnostic evaluation, red-flag presentations and modern management.

The paper proposes the SR VAIDYA Integrative Framework for Metabolic Wellness Education, built on early screening, verified diagnosis, individualised lifestyle support and responsible Ayurvedic integration. Ayurvedic and Panchakarma approaches are discussed within clear safety boundaries as complementary wellness measures that must never replace or delay evidence-based diagnosis or treatment of metabolic disease.

Introduction

Metabolism encompasses the entire set of biochemical processes by which the body converts food into energy, builds and repairs tissue, and regulates storage of surplus energy as fat.

Metabolic health reflects the efficient and balanced functioning of these processes across multiple organ systems — the pancreas, liver, adipose tissue, skeletal muscle, and the central nervous system — working in coordination.

The term “slow metabolism” is frequently used in public discourse to explain weight gain or fatigue. Scientific understanding requires greater precision: basal metabolic rate varies modestly between individuals, and clinically significant metabolic dysfunction is far more often driven by the cluster of conditions collectively termed Metabolic Syndrome than by an isolated difference in resting energy expenditure.

The Five Components of Metabolic Syndrome

Under the widely used Joint Interim Statement (JIS) and International Diabetes Federation (IDF) consensus, a diagnosis is established when an individual exhibits at least three of the following five criteria:

COMPONENT 1

Elevated Waist Circumference

Central (abdominal) obesity based on population- and ethnicity-specific thresholds (the strongest single predictor).

COMPONENT 2

Elevated Blood Pressure

Systolic BP ≥130 mmHg and/or Diastolic BP ≥85 mmHg, or receiving drug treatment for hypertension.

COMPONENT 3

Elevated Fasting Glucose

Fasting plasma glucose ≥100 mg/dL (5.6 mmol/L), or receiving pharmacological treatment for elevated glucose.

COMPONENT 4

Elevated Triglycerides

Serum triglycerides ≥150 mg/dL (1.7 mmol/L), or receiving specific drug treatment for elevated triglycerides.

COMPONENT 5

Reduced HDL-C

HDL cholesterol <40 mg/dL in men or <50 mg/dL in women, or on drug treatment for low HDL.
